Make a form

Build sign-up forms that grow your list, and questionnaires and waivers people fill in when they book.

Forms are every place a visitor fills something in. The Forms page has two kinds, on two tabs: Sign-up forms collect emails and can start your automated emails, and Questionnaires & waivers ask questions when someone books or anywhere you share them.

Make a sign-up form

  1. Open Marketing > Forms and stay on the Sign-up forms tab.
  2. Press Create a form and choose a type: Pop-up, Banner, Inline, Full page, Link in bio, Video, Countdown or Spinner.
  3. Write the headline and supporting line, and choose what people fill in. Email is always included.
  4. Under Organize these contacts with a tag, add a tag. Under Send these automated emails, pick a funnel if you want one to start.
  5. Press Publish form, or Save as draft to finish later.

Make a questionnaire or waiver

  1. Open the Questionnaires & waivers tab.
  2. Type a name in New form name and press New form. Make as many as you need.
  3. Add an Introduction if you like. It shows above the questions.
  4. Press Add question and choose a type: Short text, Long text, Choose one (dropdown), Choose one (buttons), Checkbox, or Consent / waiver. Switch on Required where you need an answer.
  5. Press Save form.
  6. Attach the form to a gathering or a 1:1 session type so people answer it when they sign up. Or switch on Publish this form to give it its own web address.

Share a published form

  • Under Link to the form, copy the link and send it to anyone.
  • Under Put it in a website, press Copy embed code and paste it into any website, even one not built in ColibriOS. On a ColibriOS page, add a Form section instead.

Good to know

  • Everyone who sends a published form becomes a contact, and their answers sit on their profile in Contacts. The Responses card on the form lists every answer, with Download CSV. You also get an email each time.
  • For a waiver, use the Consent / waiver question. People must tick it before they can finish.
  • Switch on Ask every time for a question that must be answered fresh at every booking.
